1. Who we are

UROMAX is a physician-led medical service operated at LeafMedx Medical Centre, #108, 6202 29 Ave, Beaumont, Alberta T4X 0H5. This policy explains how personal information collected through this website is handled.

UROMAX complies with Alberta's Personal Information Protection Act (PIPA) and, where applicable, Canada's federal Personal Information Protection and Electronic Documents Act (PIPEDA). Where information collected through this website becomes part of your clinical record, it is also governed by Alberta's Health Information Act (HIA) and the custodian obligations that apply to it.

2. Information we collect

Contact and booking information: the name, phone number, email address, and any message content you submit through the consultation request form or the appointment booking flow.

Booking and payment metadata: appointment selections and confirmation of a scheduling fee payment. Card details are processed by our payment processor and are not stored on this website.

Technical information: standard server logs (IP address, browser type, pages visited) maintained for security and site operation. Analytics data is collected only with your consent, as described in Section 7.

Please do not send detailed medical information through website forms. A private consultation is the appropriate place to discuss your health history.

3. How we use information

Information collected through this website is used to respond to your consultation request or booking; to schedule, confirm, and administer appointments; to communicate with you about your enquiry or appointment; to maintain the security and operation of the website; and to meet legal and regulatory obligations applicable to a medical practice.

We do not sell your personal information, and we do not use it for purposes unrelated to the services described above without your consent.

4. Disclosure

Personal information is shared only with service providers who support the operation of this website and the clinic (for example, form delivery, appointment scheduling, and payment processing), and only to the extent needed to provide those services. Information that forms part of your clinical record is disclosed only in accordance with Alberta law — including HIA rules governing health information — or with your consent. We do not disclose personal information to your family physician or any other party unless you request it or the law requires it.

5. Retention

Enquiry and booking information is retained only as long as necessary for the purposes above and to meet legal and regulatory record-keeping obligations that apply to medical practices in Alberta. Information that becomes part of your clinical record is retained in accordance with HIA and College of Physicians and Surgeons of Alberta record-retention requirements.

6. Security

We use reasonable administrative, technical, and physical safeguards appropriate to the sensitivity of the information, including encryption of data in transit. No method of transmission over the internet is completely secure, and we cannot guarantee absolute security.

7. Cookies and analytics

Essential cookies are required for the website to function. Analytics tools load only after you provide consent through the cookie banner. If you decline, non-essential tracking tools are not loaded. You can change your choice at any time by clearing site data or using your browser settings.

8. Your rights

You may request access to the personal information we hold about you, ask for corrections, or withdraw consent to future collection, use, or disclosure (subject to legal and contractual restrictions and to record-retention obligations that apply to clinical records). To make a request, contact the clinic at 780-737-5773. If you have a concern about how your information has been handled, please contact us first; you may also contact the Office of the Information and Privacy Commissioner of Alberta.
